Nominations/Submissions Open

The Foundation for Post-Acute and Long-Term Care Medicine is now accepting nominations and submissions for the following programs.

Quality Improvement Award
One project that supports a quality improvement project will be selected for this $5,000 award. This program is designed to encourage the development of innovative projects that will help to make a distinct impact on the quality of PALTC. The award will support initiatives that focus on facility staff education, quality improvement programs, research on interventions and treatment, and health literacy to directly enhance the quality of care provided to patients in long term care settings. Quality Improvement and Health Outcomes Awards
Up to three awards of $1,000 each will be awarded to facilities that have implemented programs that improved the quality of life for their residents. These prestigious awards are based on programs medical directors and care teams have implemented and demonstrated to improve the quality of life for their PALTC residents. Marian McNamara, RN, MSN, and her team at Sea View Hospital Rehabilitation Center & Home in Staten Island, NY, won the Foundation’s 2017 QIHO Award for their project, “Enhanced Palliative Care.” “I think we’ve done a fabulous job. And we are very proud of our accomplishment. It meant a great deal to be recognized by the Foundation. It helped us to move this program forward and share it with others nationwide,” says Ms. McNamara. At the same time, the award was important validation for people who work hard but sometimes may feel unappreciated. “They do so much for our residents, and awards like this are important validation that their tireless efforts are appreciated.”

James Pattee Award for Excellence in Education
Established in 1997 to honor to James Pattee, MD, CMD, the father of the CMD Certification Program and the Core Curriculum program, as well as a founder of the Society, this honor is awarded each year at the Society’s Annual Conference to an individual in recognition of his/her significant contributions to the educational goals of the Society, as well as to advance education specific to PALTC practice, and to promote leadership via educational endeavors within the PALTC continuum. “The award had a huge impact on me. My family was there, which was important to me; and some of my colleagues were in attendance,” says 2017 Pattee Award winner Matt Wayne, MD, CMD. “It was wonderful to have them all there to be a part of this. It made me proud. Any recognition is nice, but getting it from colleagues is one step above.”

 

 

 

Medical Director of the Year Award
The Medical Director of the Year Award recognizes those individuals whose vision, passion, leadership, knowledge, and commitment succeed in taking patient care in the facilities they serve as medical director to exceptional levels of quality, excellence, and innovation. William Dodd Founders Award
Named for the founder of the Society, this award recognizes significant contributions by those individuals who build the organizational strength, image, and mission of AMDA-The Society for Post-Acute and Long-Term Care Medicine to promote medical direction and physician services in PALTC by enhancing the reputation of the Society, and advancing the Society's goals to improve the care delivered to patients throughout the PALTC continuum.
