FUTURES PROGRAM – A WORKFORCE DEVELOPMENT INITIATIVE

Upon completing the Foundation Futures Program, participants should be able to:

• Recognize the variety of ways to practice in the PALTC setting.
• Delineate the roles and responsibilities of providers/medical directors who serve in PALTC settings.
• Depict the contributions of the interdisciplinary team.
• Describe components of PALTC settings, including clinical capabilities, financing, regulation, commonly used terms, and acronyms.
• List the benefits of AMDA membership, such as professional community participation, advocacy for setting, practice, and patient-specific needs, and CME/CE opportunities in engaging learning formats.

• PGY IV or V Geriatric Medicine/Geriatric Psych Fellows
• PGY II or III Internal Medicine or Family Physician Residents
• Advanced Practitioners: Advanced Nurse Practitioners, Physician Assistants including those completing their training
• Allied Health professionals completing their training
• Previous participants are not eligible to apply

Participants will receive the following benefits:

• Admission to the Futures Program on Saturday, March 9, 2024. Attendance at this all-day event is required.
• Registration to PALTC24 – AMDA’s annual conference from March 10-11, 2024, in San Antonio, TX. Daily attendance at PALTC24 is required.
• One-year membership to AMDA-The Society for Post-Acute and Long-Term Care Medicine beginning January 2024. Membership benefits include a subscription to JAMDA -the Society’s scientific peer-reviewed journal online, and access to members-only content via podcasts and AMDA’s APP.
• Participants will have access to resources provided through Futures Connect, the year-long online program that offers platforms for networking, discussions about topics of clinical interest, and open discussions with PALTC leaders.

The 2024 agenda is not yet available.

Please prepare the following items to upload with your online application:

• Letter of Interest (LOI) – Your LOI should be no more than 250 words and describe your interest in long-term care practice and why you feel you will benefit from participating in this program. 
• Curriculum Vitae (CV)
• A current high-resolution photograph 

Read and accept the Futures Program Policies before submitting the online application. Submission of your Futures Program application indicates that you have read and accepted the Futures Program Policies.

 

A Letter of Support (LOS) by the applicant’s program director or another AMDA member must be emailed to Futures@paltc.org following submission of your online application. Your application will not be complete until we receive the LOS.

Cancellation Policy
The purpose of this policy is to provide a cancellation procedure for Futures participants, should a cancellation be necessary.

 

Statement of Policy
If a Futures participant would have to decline or withdraw from the Futures Program, they must contact AMDA’s program manager immediately by phone and in writing. The participant’s program director will also be notified of the cancellation.

 

Travel Policy
This policy aims to ensure consistent procedures among Futures participants awarded funding for travel and hotel expenses from the Foundation for PALTC Medicine; it does not apply to those whose expenses are paid by another organization or the participant.

 

Statement of Travel Policy

1. Travel arrangements, including changes and cancellations of travel arrangements, can be made directly by the Futures participant. Flight changes and cancellation fees will be the responsibility of the Futures participant.
2. The lowest available airfare shall be sought when booking. All travelers must use economy class.
3. If the conference location is within driving distance of the participant, the participant will be reimbursed for mileage (federal rate) and parking fees.
4. AMDA – The Society for Post-Acute and Long-Term Care Medicine’s Annual Conference registration will be paid directly by the Foundation.
5. Staff will make the hotel accommodations in AMDA’s annual conference room block. If a participant receives funding and requires private accommodations, the Foundation will provide two nights of hotel accommodations, and the participant will be responsible for additional nights.
6. Futures participants will be responsible for the costs of ground transportation to/from the conference and any food costs (breakfast and lunch will be provided on the day of the  Futures Program).

Many educational programs or employers provide funds to attend a professional conference. If you need funding for travel/lodging expenses, you may submit a Request for Funding when you complete the application. Please note that there are limited funds available.

The Futures Program seeks to expose residents, fellows, and advanced practitioners to the numerous career opportunities available in post-acute and long-term care (PALTC) medicine through events. In 2023, 87 practitioners participated in the program.

To maximize their opportunities while at PALTC24, we are looking for experienced practitioners to answer questions and share their experiences with a Futures participant. You will be paired with a Futures participant before you arrive at PALTC23 so you can make arrangements to meet during the conference to offer guidance and networking opportunities. Additional information and suggestions will be sent to you as we get closer to PALTC24.
