WHOLE Communication: A High Reliability Organization Approach to Post-Acute-Long-Term-Care Education

The aim of this project is to reduce patient risk through effective and efficient communication amongst interdisciplinary teams working with Veterans who are in need of Post-Acute-Long-Term Care (PALTC). High Reliability Organization research argues that communication is critical for care planning (Tolk, 2015). Inefficient and silo communication can lead to longer wait times and result in incomplete referrals when applying to facilities. When communication is conducted with the whole team of stakeholders present providers have clearly defined roles and feel heard in being able to voice their contributions as a part of the patient's care plan and the referral process.
