As the population ages, heart failure is becoming increasingly prevalent among older adults. Despite advances in medical therapy, heart failure remains associated with high morbidity, mortality, and hospitalization rates. In the U.S., it accounted for 45% of cardiovascular deaths in 2022, with mortality rising over the past decade. Hospitalizations reached 1.2 million in 2021 and continue to increase, with total heart failure–related costs projected to reach $858 billion by 2050. Heart failure patients in skilled nursing facilities (SNFs) are typically older, frailer, and more clinically complex, with 30-day rehospitalization rates ranging from 27–43%.
