QAPI Alert — Do We Still Need to Discuss the Call Bell?

Dear Dr. Diane: I have a situation that keeps occurring during our QAPI meeting. The administrator is asking that we conduct another call bell performance improvement plan. But the issue is that this building continuously has a problem with call bells; we solve it for one unit and then another complaint on another unit triggers this conversation. Every month it’s call bells. I am frustrated, and I wonder is it something wrong with this facility? The administrator? Or am I wrong?
