A Pragmatic Approach to Identifying Goal-Concordant Care for Nursing Home Residents With Alzheimer's Disease or Related Dementias

People living with Alzheimer's disease and related dementias (ADRD) experience treatments that are burdensome, costly, and often do not align with their goals.1 Although receipt of goal-concordant care is considered a criterion standard outcome of patient-centered care in serious illness, it remains difficult to measure, especially in pragmatic clinical trials in which outcomes are typically ascertained using real-world data from administrative or electronic health record (EHR) sources.2 We sought to develop an innovative, pragmatic approach to identify goal-concordant care among nursing home (NH) residents with ADRD using existing EHR data from one large NH chain for future use in pragmatic trials.
