Patient Priorities Care in Post Acute and Long Term Care

Healthcare of individuals with multimorbidity can be challenging, as specific disease-focused care may bring unintentional harm to patients with multiple chronic diseases. In Post Acute and Long Term Care (PALTC), the residents' goals and preferences must guide their care. The Patient Priorities Care (PPC) approach (https://patientprioritiescare.org/) helps elicit individuals' goals and preferences and the tradeoffs they are willing to make to achieve their high priority goals. In this case, the provider used the PPC, an emerging approach to guide the patient in making healthcare choices and identifying tradeoffs.
