Palliative Care Services in a Nursing Home Setting-Influence on Care Preferences

Preventing hospital readmissions while meeting patients’ needs is one of the top priorities of subacute facilities. Our experience suggests that for many older patients with multiple comorbidities, the hospital stay adds little to their well-being and subjects the patient to the burdens of hospital care. We report results of palliative care (PC) program at our nursing facility.
