Room changes within the facility, outpatient appointments, and recreational trips occur frequently in post-acute and long-term care but are often overlooked as processes benefiting from clinical oversight. In an effort to call attention to the level of coordination warranted, the Transitions of Care Subcommittee of the Post-Acute and Long-Term Care Medical Association (PALTmed) has taken a special interest over the past three years in reframing these trips and room changes as care transitions and has coined the term “microtransitions” to describe their often brief and non-urgent status.
