‘High quality transitional care’ is the safe and efficient transfer of patient care across health settings. It is important not only to prevent harm but also to reduce unnecessary medical expense. Previous studies have found that medication reconciliation errors play a significant role in impacting ‘High quality transitional care’. This study investigated the prevalence of medication reconciliation errors among patients discharged to Skilled Nursing Facilities (SNFs) and whether these medication reconciliation errors impacted 30-day readmission.
