For patients who are at higher risk of thromboembolic events, the advent of direct oral anticoagulants (DOACs) was a major breakthrough.1 With the aging population, an increasing number of older patients with multiple comorbidities, concomitant drug use, and frailty are likely to receive DOACs. However, recent findings suggest that caution is warranted as higher than expected rates of bleeding were observed in frail older patients who were switched from vitamin K antagonists to DOACs.2
