The Implementation of an Advanced Care Planning (ACP) Template on Quality of ACP Visits Performed by Advanced Practice Providers

Advanced care planning in the PALTC setting can improve quality of life and decrease use of unwanted medical services. Research of best practices for ACP is ongoing, and there is a need for strategies to improve the quality and uptake of this service. Providers should be prepared to address these issues and properly document their outcomes.
