Approximately 20% of hospital discharges for heart failure (HF) in the United States result in an admission to a skilled nursing facility (SNF). Patients discharged to a SNF with HF are usually older, frailer, with more significant comorbidities with higher mortality risks than those who go home after hospitalization. Thirty-day re-hospitalization rates for HF from SNF range from 24% to 43%. A study suggested that Medicare spent $3.39 billion on potentially avoidable SNF hospitalizations. This project aimed to assess SNF staff's comfort and knowledge in treating HF patients before and after an educational intervention.
