Is Health Information Exchange Participation Associated With Hospital Readmissions From Home Health Care?

Adults discharged from the hospital with skilled home health care (HHC) are at high risk for preventable adverse events, including medication errors and hospital readmissions.1–3 Health Information Exchanges (HIEs) are increasingly being used in multiple care settings across the United States.4 HIEs are designed to allow clinicians in different care settings to access a patient's medical information electronically, and have potential to improve information exchange between hospitals and other care settings.
