Interdisciplinary teams (IDT) play a vital role in long-term care (LTC) for patients with complex medical conditions, coordinating assessments and treatments across various health care disciplines. In LTC, IDT teams comprise professionals including, but not limited to, primary care providers, behavioral health providers, social workers, nurses, administrators, case managers, directors of nursing, assistant directors of nursing, and therapy providers, who collaborate to address the diverse medical needs of residents.
