We wish to respond and contribute to the important discussion in the timely letter by Sakurai et al., “Should Frailty Be Subdivided in Definition and Assessment?”1 highlighting the growing use of domain-specific terms, such as “oral,” “cognitive,” and “social frailty.” The authors rightly caution against the potential fragmentation of the broader concept of frailty and we share their emphasis on preserving a holistic perspective. Furthermore, we offer several additional considerations that reflect the conceptual, cultural, and ethical complexities surrounding frailty.
