Skilled nursing facilities (SNFs) are faced with providing care for residents with increasing clinical complexity. Residents are being discharged from hospitals to post-acute care earlier and with multiple active co-morbidities. Clinicians working in the SNF are charged with delivering high-quality medical care in the facility and reducing avoidable re-hospitalizations. Initially developed for use in the acute care setting, contact-free continuous patient monitoring (CFCPM) is a system designed to continually collect vital signs and other clinical parameters.
