Diabetes Management in Post-Acute Care: A Case Study

Managing glucose levels can be challenging in the post-acute and long-term care (PALTC) settings. It becomes even more complex for those who are undergoing rehabilitation following a recent surgery. Temporarily held medications, changes in nutritional and functional status, and surgical stress can worsen perioperative glycemic control. Post-acute care diabetes management aims to maintain optimal glycemic control while transitioning to a sustainable home regimen. This case study presents an older adult undergoing post-surgery rehabilitation and details how the PALTC team managed his diabetes and transitioned to home.
