Developing a Screening Tool for Palliative Care Referrals on Admission to Post-Acute Rehab

Many patients use the post-acute rehab (PAR) benefit during the last year of life, and a significant number of seriously ill patients die in PAR. PAR patients are often medically fragile and at risk for decompensation. This suggests urgent needs for both increased palliative care (PC) services and advance care planning (ACP) in the PAR population.
