Code Status Clarity: A Multidisciplinary Approach to Advance Care Planning

Starting in February 2023, by October 2023 the code status of patients will be easily identifiable in the electronic medical record across our practice in the post-acute, long-term care, and assisted living setting. With our almost 4,000 patients across three states 95% will have code status clarified.
