CMS Requirements for Hospital Discharges to Post-Acute Care Providers

Last week, the Centers for Medicare & Medicaid Services (CMS) posted a memo regarding requirements for hospital discharges to post-acute care providers (QSO-23-16-Hospitals). CMS identified areas of concern related to missing or inaccurate patient information when a patient is discharged from a hospital into the post-acute and long-term care setting. This memo reminds state agencies, accrediting organizations, and hospitals of the importance of regulatory requirements for discharges/transfers to post-acute care providers. It includes recommendations that hospitals can use to improve their discharge policies and procedures.
