Beyond Frailty: Therapeutic Inertia After Transition to Long-Term Care

We commend Stasinopoulos et al1 for their timely and clinically important analysis of changes in glucose-lowering medication (GLM) use following hospital discharge to long-term care facilities (LTCFs). This study provides valuable insight into prescribing patterns during a critical transition in care for older adults with type 2 diabetes, a population characterized by frailty, multimorbidity, and polypharmacy.
