Actinomyces Odontolyticus Bacteremia in an Immunosuppressed Patient

Mr. X is an 81-year-old male with history of atrial fibrillation, meningioma, and recent pacemaker placement 1 month prior reporting to the hospital after being called by the ER for positive blood cultures. Apparently, he was in ER 4 days prior with fever, chills, malaise, and shortness of breath, and blood culture and urine cultures were done. He was discharged with a diagnosis of bronchitis and given azithromycin and steroids. He had regular dental cleaning 3 weeks prior without any antibiotics prophylaxis.
