Columbia, MD – Sabine von Preyss-Friedman, MD, CMD, chief medical officer of Washington state-based Avalon Healthcare, has been named the 2021 Choosing Wisely® Champion by AMDA – The Society for Post-Acute and Long-Term Care Medicine. The award honors individual clinicians and teams of clinicians who have gone above and beyond to make significant contributions to advance the ideals of the ABIM Foundation’s Choosing Wisely campaign by working to reduce unnecessary tests, treatments, and procedures in health care.

Dr. von Preyss-Friedman, along with Kathy Owens, MSN, Avalon senior vice president of clinical services, and senior Omnicare pharmacist consultant Alan Bell, RPh, implemented recommendation #2: “Don’t use sliding scale insulin (SSI) for long-term diabetes management for individuals residing in the nursing home.”

In their project, Dr. von Preyss-Friedman and her team had an initial goal to reduce the utilization of SSI by 25% within one year. They started in September 2019 with a total of 346 SSI (33.1% of diabetic population). By September 2020 they exceeded their goal. Then they had a total of 137 SSI (16.4% of diabetic population), representing a 48.2% reduction of SSI within a year. The team found that the reduced need for nursing to handle an SSI saved time and money as well.

At this point, says Dr. von Preyss-Friedman, “Every SSI is reviewed at the facility level by the IDT on a regular basis and questioned after an initial post-acute period of 1-2 weeks, resulting in significantly less trauma to diabetic patients, decrease in nursing time on a potential harmful procedure, better quality of life for our diabetic patients, and likely less hypoglycemic episodes.”

The team was “ecstatic to receive this award and to be recognized in this way, especially during the pandemic when we’ve been through so much. We are even more excited about what we were able to achieve for our residents,” Dr. von Preyss-Friedman says. “It’s been win-win-win,” she notes, with better outcomes, less discomfort for patients, and more efficient use of nursing time.

Choosing Wisely is so important, she stresses. “It identifies things that have been done for so long that don’t provide any benefit. It also points out the best clinical practices, and it’s our responsibility to provide these. They are applicable and relevant to our patient population and lead to better wellbeing, health, and outcomes to these vulnerable individuals.”

Launched in 2012 by the ABIM Foundation, Choosing Wisely is widely recognized across the health care system as a leading effort to reduce overuse in medicine. More than 80 specialty societies, including AMDA, have joined the campaign and published over 600 recommendations regarding overused or unnecessary care.
