Quality Improvement Project to Enhance Knowledge of Hospice Interdisciplinary Team of Post-Acute Care Services

Hospice services provided to residents of Post-Acute and Long-Term Care (PALTC) facilities are associated with higher resident and family satisfaction and lower health care cost. One barrier to timely hospice enrollment is limited understanding of hospice services and referral procedures by the PALTC interdisciplinary team (IDT). In part 1 of this 2-part Quality Improvement (QI) project, the project's PALTC medical director and leadership team designed and implemented a project to enhance the knowledge of the PALTC IDT of hospice services and referral processes.
