Medication Errors Upon Hospital Discharge to Skilled Nursing Facility

Discharge documents are the main means of communication between healthcare settings, and their transmission from hospital to skilled nursing facility (SNF) is essential to assure high-quality care transitions, provide optimal patient care, and prevent unnecessary emergency department (ED) visits and re-hospitalizations. The accuracy of discharge documents from the hospital to SNFs is unknown. Studies have reported high rates of inaccuracies in medication documentation on hospital discharge summaries when transferring to SNFs.
