Following an acute hospital stay, 1 in 4 Medicare patients is transferred to a skilled nursing facility (SNF).1 Meanwhile, 25% of these patients are readmitted to the hospital within 30 days.2 In addition to the increasing costs, repeated hospitalizations and multiple transitions of care put patients at risk for adverse health outcomes, including decline in physical function, medication errors, and delirium.3,4
