BE IT RESOLVED, that the American Medical Directors Association request that the Centers for Medicare and Medicaid Services amend the Medicare Conditions of Participation for hospitals to require a medication profile (that includes indication, goals, and plans for monitoring towards goals consistent with compliance with F Tags 428 and 329) accompany the patient when there is a discharge to a nursing facility.

THEREFORE BE IT RESOLVED, that AMDA the Society for Post-Acute and Long- Term Care advocates for a waiver process for nursing homes to provide medically and regulatory necessary video conferencing telemedicine visits with a physician, including current regulations for delegation of tasks to advanced practice providers, for the purpose of providing primary care to the nursing home resident, when the nursing home has demonstrated due diligence in attempting to secure a physician. The waiver will be for period of one year, renewable annualxly;

THEREFORE BE IT RESOLVED, that AMDA-The Society for Post-Acute and Long- Term Care Medicine, together with likeminded professional organizations such as American Geriatrics Society, American Medical Association, and consumer organizations such as AARP and others, advocate for legislative action to create a pathway to immigration for undocumented noncitizens in the United States, who show their commitment to their intended homeland by working as Certified Nursing Assistants and/or Nurses in Post-Acute and Long-Term Care settings for a minimum of five years.
