Heart failure (HF) is a clinical syndrome associated with high hospitalization rates and poor outcomes, particularly in older adults.1 Although structured postdischarge follow-up is guideline recommended,1,2 real-world evidence on telehealth-supported management remains limited. We therefore evaluated the association between a structured telemedicine (TM) follow-up program and clinical outcomes in older adults discharged after hospitalization for HF.
