Advance Care Planning Pop-Ups: Feasibility and Efficacy among Older Adults

Advance care planning (ACP) is the process of documenting future healthcare preferences prior to incapacitating illness. The Illinois Department of Public Health provides four advance directive documents (POA, Living Will, POLST, MHTD) and completion of these can improve care concordance with patient wishes. While ACP has been shown to improve care concordance and patients report desiring conversation surrounding end of life wishes, ACP uptake remains low among older adults. The ACP "Pop-Up" program aims to increase ACP awareness and completion of advance directive documents among older adults in Chicago.
