Advancing Education on Advanced Care Plan Discussions for Clinicians

Objective: to improve quality of ACP discussions, the confidence in clinician's ability to conduct ACP discussions, and documentation of ACP discussions. Who: Primary Care Providers (NPs and PAs) in nursing homes who have already undergone scripted regimen of training including prognosis training and training grounded by the VitalTalk's education around Serious Illness Conversations Intervention: Three 30-minute education lectures with discussion were created to compliment the providers' previous education and experience through collaborative teaching.
