Scaling Up a Palliative Program in Long-Term-Care Across Canada

The aim of this initiative was to increase the uptake of an evidence-based palliative program in long term care (LTC) homes across Canada to address the growing population of residents with complex, progressive conditions, to support compassionate, person-centred care.
