Strengthening Fall Injury Reduction in Long-Term Care: 3 Implementation Enhancements to the 2025 PALTmed Falls CPG

The PALTmed Clinical Practice Guideline (CPG) on falls and fall prevention in the post-acute and long-term care setting represents a timely and rigorous contribution to the field.1 Its 10-step care delivery framework, alignment with the 4 Ms model of age-friendly care, and comprehensive attention to multifactorial risk assessment give clinical teams a practical, evidence-grounded foundation. I write to propose 3 targeted implementation enhancements for the CPG's framework that, if adopted, would substantially strengthen its effectiveness in reducing fall-related injuries in long-term care (LTC) residents—which is the more achievable and clinically meaningful outcome measure than fall reduction alone.
