Recommendations for COVID-19 Vaccine in Post-Acute and Long-Term Care Residents and Staff

This statement provides recommendations for use of the 2025-2026 COVID-19 vaccine in residents and staff of post-acute and long-term care (PALTC) facilities. Residents of PALTC settings are predominantly older than 65 and have multiple comorbidities, placing them at high risk for severe illness from COVID-19.1,2 Staff, both clinical and nonclinical, are also at heightened risk of exposure to COVID-19 in PALTC settings.3 Although data on prevention of transmission are limited, vaccination may help reduce transmission between staff, residents, and residents’ visitors, as well as staff and their household contacts.
