In 2026, the Futures Program will celebrate its 25th anniversary — a milestone that reflects both its enduring impact and its vision for the future. Launched by the Foundation for Post-Acute and Long-Term Care Medicine in 2001 to address the growing shortage of advanced practitioners in PALTC, the program has become a cornerstone of workforce development in the field. It is now a joint program between the Post-Acute and Long-Term Care Medical Association (PALTmed) and the Foundation. What began as an initiative rooted in medical direction has steadily evolved, expanding to embrace advanced practitioners and highlight the collaborative, interdisciplinary nature of PALTC medicine.
