Long-term care (LTC) residents experience a high burden of unmet needs and distressing symptoms near the end of life and are subjected to potentially harmful transfers to hospital. Advance care planning (ACP) aims to align resident care received at the end of life with their goals and wishes. However, ACP for LTC residents remains suboptimal.1,2
