Pharmacist-Mentored Implementation Collaborative: High-Risk Medications Stewardship Program

By November 2024, the Pharmacist-Mentored Implementation Collaborative (PMIC) will reduce the incidence of adverse drug event-related harm in nursing homes in the IPRO-QIN-QIO region for the categories of anticoagulants, diabetes, antimicrobials, and opioids. Through data analysis, deployment of toolkits, education of consultant pharmacists and the nursing home staff, the number of ADEs will be reduced by 15%, and the number of ADEs resulting in harm will be reduced by 15%.
