Skilled nursing facilities (SNFs) serve as vital hubs for in-patient rehabilitation and medical care for patients in need of temporary residential care. While SNFs primarily aim to facilitate the transition of patients back to the community, some individuals require continuous custodial care. This study explores the social characteristics of patients discharged from SNFs to the community.
