Care transitions are common in older adults in post-acute and long-term care, and managing diabetes during care transitions between hospitals and skilled nursing facilities is fertile ground for unintended negative outcomes (Endocr Pract 2021;27: 370–377). Diabetes is a highly prevalent chronic condition: 14.7% of the U.S. population (37 million) have diabetes, and the prevalence is much higher at 29.2% among adults 65 and older (“National Diabetes Statistics Report,” Centers for Disease Control and Prevention, 2022, https://bit.ly/47WFqrk).
