Abnormal Cognitive Function Screening and Subsequent Dementia (ADRD) Diagnosis After Heart Failure Hospitalization: Implications for Transitions of Care

Consequences of Alzheimer’s disease and related dementias (ADRD) complicate care of patients with heart failure after hospitalization. Skilled Nursing Facility (SNF) admission with routine cognitive assessment could help guide care planning. However, the association of SNF cognitive screening and subsequent ADRD diagnosis remains unknown. This study will determine how often new ADRD diagnosis follows abnormal cognitive screening in Veterans with heart failure admitted to SNF.
