AMDA resolves to work with HCFA to clearly display, on Durable Medical Equipment (DME) forms requiring physician signature for approval, the charge of the equipment being dispensed; and

AND BE IT FURTHER RESOLVED, that AMDA work with HCFA to display, on rental DME forms requiring physician signature for approval, the monthly rental charge, the cost of the equipment, and the length of the lease.
